LACTATION CANCELLATION POLICY: Appointments cancelled with less than 24 hour notice will be charged a %50 of fee. Clients who cancel with less than 2 hours notice will incur a fee of 75% of service fee. For No Shows, there will be a charge for $50 (virtual) & $150(in person). A "No Show" is defined as a client who has not shown up for their visit within 15 minutes of their scheduled appointment time either virtually or in person. ALL fees for services are non refundable. The payment and no refund policy is SEPARATE from the cancellation policy. Each client is responsible for all fees associated with each consult. Payment is due at the time of appointment and an invoice will be issued to you. By scheduling your consult, you grant me permission to communicate with your insurance company regarding the services provided to you and to your child(ren). POSTPARTUM DOULA CANCELLATION POLICY: Best Latch- Doula Services will make every effort to be with the Client at the scheduled times. If she is not able to come due to sickness or other emergencies (including attending another urgent client if she is on call.) The Client(s) will be informed in advance. If Best Latch- Doula Services cannot work the agreed hours on any given shift, she will notify the client or the agency & will make every effort to provide a qualified backup. The client will be responsible for payment of the back up services services. The backup postpartum doula will provide the same services outlined in this contract. Client will also be given the option to reschedule those hours if Client prefer. Client can cancel or reschedule scheduled postpartum shifts 24 hours in advance. Deposits made (if applicable) are always non refundable. Client cancelling or rescheduling more than 24 hours in advance will not incur any additional fees, although deposits are non-refundable and non-transferable. Client cancelling or rescheduling less than 24 hours before a shift, but more than 2 hours before a scheduled shift will incur a fee of 1 hour of service as designated below, based on the time of the scheduled shift. Client cancelling or rescheduling less than 2 hours before a scheduled shift will incur a fee of 2 hours of service at the rate designated below, based on the time of the scheduled shift. In the event of illness or emergencies for either party or severe weather, every effort will be made to reschedule services at another time. If any payment for that day has already been made, it will be applied to future service.
